The Health District presents itself to Primary Care Physicians

The District should be understood as the Theater of Primary Care in all its aspects. The actors are:

  • General Practitioners;
  • Free Choice Pediatricians;
  • External Specialist Doctors;
  • Employees in the Medical Area and the Health Professions;
  • Social Services of Municipalities;
  • Voluntary associations

Services/Activities provided Direction District A

The Services provided by District A Management are as follows:

  • Relationship with all institutions (Municipalities, Associations, etc.);
  • Relationship with General Practitioners and Paediatricians of Free Choice;
  • PMA authorisation outside the Lazio Region;
  • Issuance of authorisations for extra-tariff devices/aids, for rare diseases, cystic fibrosis;
  • Authorisations for the release of Micropumps and accessory material for users with diabetes mellitus;
  • Request for contribution for adaptation of vehicles driven by disabled people or intended for the transport of disabled people;
  • Authorisation for Radiotherapy/Hydrotherapy at affiliated private centres;
  • Reimbursement pursuant to art. 26;
  • Reimbursement of users undergoing dialysis treatment;
  • Refunds for various reasons (Nursing Homes, Fiuggiterme spa, etc.);
  • Repayment of transplanted or waiting list Regional Law 41/2002;
  • Payment of ADI Service Companies.

In view of the termination of MMG Dr. Rosa Anna Maria, from 01/10/2021, we inform you that from the aforementioned date a provisional assignment has been conferred that cannot be assigned to Dr. GABRIELE Francesca, who will carry out her activity at the medical office located in Guarcino – Via Milani No.1, with the following schedule:

UDI (Nursing Inpatient Unit)

The Intermediate Care Area with high nursing care intensity is equipped with residential nursing management places where medical care is ensured by General Practitioners or other doctors affiliated with the S.S.N.

The taking charge is aimed at those patients who need:

  • Health interventions that can potentially be delivered at home but need temporary hospitality, for the time necessary to guarantee suitable conditions (structural and family) for effective social and health care for:
    • Patients, without age limits, characterized by non-self-sufficiency, even temporary, in the phase of the disease in which a high technological commitment and continuous medical presence/assistance are not required, such as in the acute care hospital;
    • Patients with pathologies such that they cannot be adequately followed in Integrated Home Care (ADI) or in the event that it cannot be organised;
    • Patients discharged from the hospital for acute care but not adequately assisted at home due to the clinical-care complexity required or for social reasons in cases where the family or a social structure is still unable to support the patient in the disease;
    • Patients who, due to their particular social and health situation, need a diagnostic, therapeutic and monitoring path that is difficult to manage at home
  • Continuous nursing surveillance. The average foreseeable hospital stay is 15/20 days with a maximum of 30 days.
  • Assistance is guaranteed 24 hours a day by nursing and care staff (O.S.S. – O.T.A). Medical assistance is guaranteed by the General Practitioners according to the signed protocol, by the Care Continuity Doctors based in Anagni and by the Doctors of the Point of First Intervention during the night, pre-holiday and festive hours, guaranteeing assistance in emergencies that may occur.

To access it is necessary that the General Practitioner or the Hospital Physician, if the patient is hospitalized, make a proposal to the District according to the attached forms.

The patient must be discharged from the hospital acute care unit and the request will be examined within 24/48 hours prior to discharge from the UVMD at the following addresses:

The transport must not be activated before the consent of the receiving facility (UDI) and the patient must not pass through the P.P.I. but will be accepted directly from the Intermediate Care Area with high nursing care intensity.

S.M.R.E.E. (Mental Health and Rehabilitation of the Evolutionary Age) District A

The Simple Structure S.M.R.E.E. (Mental Health and Rehabilitation of the Evolutionary Age) of District A consists of three clinics:

  • Collaboration with the S.S.C.F. as part of the Childbirth Preparation Courses;
  • Socio-rehabilitative activities aimed at adolescent users and young adults with medium to severe disabilities:
    • summer stays in seaside resorts aimed at serious and medium-severe patients;
    • one-day experiential outings (outings for cognitive purposes, outings aimed at developing social autonomy, days dedicated to sport);
    • cooking workshop;
    • motor activity laboratory (basketball)
  • Diagnosis and treatment of cases of child abuse and maltreatment;
  • Collaboration with the E.E.L.L. for minors and their families subjected to proceedings by the Judicial Authority (evaluation of minors, evaluations of parenting skills, psychological support for minors, parenting support, evaluation interventions and treatment of the conflict of the couple with minor children in the event of separation, family mediation);
  • Participation in the project on family custody in collaboration with the Socio-Assistance District A;
  • Collaboration with the E.E.L.L. and the District S.A. within the framework of the P.I.P.P.I.-5 project.

The most frequent requests are related to the delay in the development of language, learning difficulties encountered in school, behavioural disorders and, on the part of the Juvenile Court, the assessments of minors and their parents.

Contribution request model for modifying driving tools or adapting vehicle for disabled transport

The request for contribution, completed on the attached form and complete with the required documentation, must be delivered in original to the Protocol office of the District concerned (based on residence) or to the General Protocol of Via Armando Fabi – Frosinone.

Below are the addresses of the protocols of the Health Districts:

  • District A (Anagni, Alatri) – Anagni Hospital;
  • District B (Frosinone, Ceprano, Ceccano) – Viale Mazzini, Frosinone;
  • District C (Sora, Atina) – Via Piemonte, snc, Sora;
  • District D (Cassino, Pontecorvo) – Via Gemma De Bosis, Cassino.

It is also specified that:

  • if the request is made by the family member of the disabled person, the latter must also attach a photocopy of their (valid) identity card;
  • contributions for changes to the driving tools concern only those prescribed by the Commission for special licences and are reported on driving licences;
  • in the event that the adaptation to the guide consists of a standard device or already installed in the vehicle (e.g. automatic transmission, power steering, etc.), it must be possible to deduce from the invoice what the cost of that component is.
